This form will allow us to get a better understanding of your expectations and music preferences. This will also allow us to accurately assign a DJ to your event, so we ask that you fill this out within a month of receiving it. Keep in mind, for Weddings you will also be provided with a copy of our full ‘DJ Questionnaire’, that contains all of the other questions that we will need to know. This from simply provides us with a “screen-shot” of your event.
